Job Description

Title: LTSS Service Care Manager

Location: McAllen, Edinburg, Corpus Cristy, Brownsville Weslaco, TX

Duration: 12 Months

Shift: 8:00 AM 5:00 PM

Mandatory OT as needed 1-2 hours daily, up to10 hours on the weekends

Description:

Training

5 weeks online

8 weeks shadowing

• Caseloads will be 20-25 members
• Follow ups by phone and email, will have to be completed for all members making sure all referrals for each member are completed
• Follow ups will be made to doctor's office, DME, etc
• Documentation will be done through Teams channel excel sheet
• Manager is available through teams and email, first point of contact will be team lead

Education

Bachelor's degree and 2 4 years of related experience

Licensure

LSW

Nice to haves :

• Home Health experience
• Case Management experience 2-10 years
• Nursing facility experience
• Bilingual
