Job Description
Title: LTSS Service Care Manager
Location: McAllen, Edinburg, Corpus Cristy, Brownsville Weslaco, TX
Duration: 12 Months
Shift: 8:00 AM 5:00 PM
Mandatory OT as needed 1-2 hours daily, up to10 hours on the weekends

Training
5 weeks online
8 weeks shadowing
- Caseloads will be 20-25 members
- Follow ups by phone and email, will have to be completed for all members making sure all referrals for each member are completed
- Follow ups will be made to doctor's office, DME, etc
- Documentation will be done through Teams channel excel sheet
- Manager is available through teams and email, first point of contact will be team lead
Education
Bachelor's degree and 2 4 years of related experience
Licensure
LSW
Nice to haves :
- Home Health experience
- Case Management experience 2-10 years
- Nursing facility experience
- Bilingual
