Job Description

Overview: The Program Specialist of Memory Care is responsible for the administration and supervision of the memory care program. Projects include: implementation of an evidence-based program to provide one-on-one assistance to caregivers of persons with memory loss; Stressbusters for persons with dementia classes; training and education initiatives for CRIS staff, community professionals, and the public about dementia. Train and supervise staff who conduct case management (Memory Care Coordinator and Aide) to help family caregivers who are caring for someone with Alzheimer's disease or a related dementia. The Program Specialist will interface directly with the grantor and evaluation teams to monitor progress toward goals, identify issues with data collection, and produce required program reports. Responsibilities: Conduct research for, ensure compliance and attainment of project goals and objectives in a timely manner developing and implementing detailed work plans. Work with appropriate staff and partners to implement, evaluate, and report on project activities; provide training and technical assistance to CRIS staff as needed to implement project activities; assure maximum organization and participation in project activities. Oversee day-to-day management of Memory Care staff including training new members, monitoring project goals and compliance, determining caseloads, and supporting other Memory Care staff with challenging cases; reporting concerns and requesting support from supervisor as needed. Take Memory Care cases as needed. Collect, analyze, and report data to monitor project outcomes; generate and present reports using standard formats and graphical summaries as requested. Submit oral and written reports on a timely basis. Coordinate the communication process to designated parties, including state, federal and local contacts, as requested on behalf of supervisor and Executive Director to explain programs, recruit participation, and include information about the program in social networking, e-mail notifications, newsletters, bulletins, etc. Develop and maintain tracking for a follow-up system for projects. In a supporting role to Supervisor and Executive Director, research, develop and implement assigned projects, coordinate project activities with appropriate staff and project partners. Identifies and remains current on community resources. Meet regularly with staff as necessary to review activities. Conduct outreach and education opportunities including Stressbusters for Caregivers of Persons with Dementia and brain health and dementia classes. Salary Range: The compensation range for this position is $28.76per hour - $48.03per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
