A Hospice Clinical Manager - Licensed Practical Nurse or Registered Nurse is a leadership position that oversees the clinical operations of a hospice program , ensuring high-quality patient care and compliance with regulations. Key responsibilities include supervising clinical staff , coordinating patient care plans with the interdisciplinary team, managing admissions and case referrals , ensuring regulatory compliance , and serving as a liaison between patients, families, and physicians. The role requires a valid LPN or RN license, strong leadership skills, clinical expertise in hospice care, and often management experience.
Key Responsibilities:
- Clinical Supervision: Managing, guiding, and supporting nursing and caregiving staff to ensure quality care delivery.
- Patient Care Coordination: Overseeing patient admissions, developing, and implementing care plans, and coordinating services with the interdisciplinary team.
- Regulatory Compliance: Ensuring that the hospice program adheres to all federal, state, and local regulations.
- Staff Development: Training, mentoring, and evaluating staff performance, and participating in hiring decisions.
- Operational Management: Analyzing operational indicators to manage census changes and staffing levels and assisting with budget planning.
- Communication: Serving as a key point of contact for patients, families, physicians, and other healthcare providers.
- Problem-Solving: Investigating and resolving patient, family, and staff concerns and addressing operational challenges.
- Record Management: Ensuring accurate and timely completion of patient documentation within the electronic medical record (EMR) system.
Typical Qualifications:
- Licensure: An active nursing license in the state where they practice.
- Experience: Prior clinical experience in a hospice or home health setting.
- Management Experience: Previous experience in a supervisory or leadership role within a clinical setting.
- Certifications: Current CPR certification.
